Getting Started
Welcome to Golematis Consulting LLC. Thank you for selecting our company to assist you. To get started, we will need the following documents (all referenced forms can be found at the bottom of this page):
1. A completed and signed Engagement Agreement (only if not represented by an attorney);
2. A copy of the judgment of divorce or separate maintenance, and copy of the property settlement agreement (if any) - be sure at least one document includes the case caption page which identifies the parties, the court and the attorneys;
3. A completed Party Information Form;
4. One completed Retirement Plan Form for each retirement plan being divided. For example, if both a Ford pension and a Ford 401(k) are being divided, you will submit two Retirement Plan Forms - one for each plan;
5. A recent account statement for each defined contribution plan being divided (e.g., 401(k), 403(b), IRA, etc.);
6. If a military pension is being divided, a completed Military Form as well as the applicable documents referenced in the form.
Once the above documents have been reviewed, we may determine that additional information is needed. If so, we will inform you of same.
IMPORTANT: Be sure to completely fill out every form. Information you do not provide may delay our ability to draft your order. Please be sure to answer ALL questions, even if you think a question is irrelevant to your case.
